Standard Semen Testing Doesn't Look For Everything

A standard semen analysis measures sperm concentration, motility and morphology.

But it doesn't routinely tell us whether potentially significant bacteria are present within the semen.

Bacterial infection or inflammation within the male reproductive tract may affect sperm quality and, in some circumstances, may also be relevant where a female partner has recurrent or persistent vaginal microbiome findings.

At Truth Fertility, we can arrange semen culture testing to investigate whether potentially significant bacteria are present and, where bacteria are identified, help determine the appropriate next steps.

Why Can Bacteria In Semen Matter For Fertility?

The male reproductive tract has its own microbial environment, and research is increasingly exploring the relationship between microorganisms, inflammation and sperm health.

Certain bacterial infections have been associated with changes in semen parameters, including sperm concentration and motility, and may contribute to inflammation or oxidative stress within the male reproductive tract.

However, detecting bacteria doesn't automatically mean that they are responsible for a fertility problem.

This is why we use semen culture as a targeted investigation, rather than recommending it routinely to every man trying to conceive.

Semen Culture And The Vaginal Microbiome

Sometimes looking at one partner isn't enough.

Certain bacteria can be present within both the female and male reproductive tracts, which means that treating a vaginal microbiome imbalance without considering the male partner may not always address the complete picture.

Where particular bacteria are identified on a vaginal microbiome test, we may recommend partner treatment without further testing in some circumstances.

In others - particularly where semen parameters are reduced, symptoms are present, or there is uncertainty about what is happening within the male reproductive tract - a semen culture may provide useful additional information.

This allows us to take a couple-focused approach to the reproductive microbiome, rather than repeatedly treating one partner in isolation.

What Happens If Bacteria Are Found?

A positive culture doesn't automatically mean antibiotics are required.

The organism identified, your symptoms, semen parameters, fertility history and your partner's results all need to be considered.

Where treatment is clinically appropriate, antibiotic sensitivity testing can help guide the choice of medication.
