Tell us your story so far
Please upload a brief description of your journey to date. A few paragraphs or bullet points are fine. It helps to include how long you’ve been trying to conceive, any pregnancies or losses, treatment you’ve had, and the main questions you’d like help with.
Upload your results
You can upload as many relevant reports as you need. These might include blood tests, semen analysis or sperm DNA fragmentation results, ultrasound reports, and letters from fertility clinics.
When uploading, use the same name you used for your booking or purchase. Please put your partner’s name in your journey summary if you’re sharing their results, and make sure you have their permission to do so.
Booked a fertility consultation or Advanced Fertility Case Review?
You don’t need to upload every report now. Please have your results to hand when we meet so we can go through them together.
Purchased Check with Verity First?
Please upload the history and results you’d like Verity to consider. She’ll look for gaps and suggest appropriate next steps. This initial review isn’t a full interpretation of your results or a substitute for a consultation.
